Could it survive? Would it sin some insurance companies? Could it get grants?

You would need a lot of people to balance of the high risk people you enroll (those not eligible for private due to the expensive nature of their conditions). Without grants flowing or funding (provided via progressive taxation), the premiums would cost a fortune.
