It's not just Cigna, we fight everyday with some insurance company because the medication we are trying to prescribe is not on the insurance companies formulary, they make us prove the patient needs the medication. The process to get adequate (not good) care is very frustrating, it boils down to the insurance company practicing medicine in some cases instead of the doctor. Sometimes they will listen to the insured though especially if their HR dept gets involved.

Around here on good days we call it overmanaged care.
