Penalties announced following fatal ammonia leak ($1.16 million)

WATERLOO --- State regulators have issued more than $1 million in penalties for two companies following an ammonia leak at a Waterloo meat processor that killed one employee and severely injured a second.

The Iowa Division of Labor Services this morning announced it has proposed $1.16 million in citations and civil penalties for Beef Products Inc. in the Aug. 31 accident at its plant that killed Elizabeth "Liza" Myers, 44, of La Porte City.

Another $88,500 in citations was issued for Modern Engineering and Piping, of Evansdale, which was working on the ammonia system at the time.

Both companies have 15 working days to challenge the citations.
