Because of legal and bureaucratic barriers, slow responses by intelligence agencies and poor timing, suspicions about Moussaoui were never raised to the highest levels of the government before Sept. 11.
Reports issued by the commission investigating the deadly attacks said a "maximum" effort to investigate Moussaoui could have unearthed his connections to al Qaeda and might have disrupted the plot altogether....

Moussaoui was the subject of a report entitled "Islamic Extremist Learns to Fly" submitted to senior CIA officials in late August 2001....
