800 tons of unsecured munitions

The issue of the unsecured Iraqi ammunition dumps has been off my radar screen; looking for information to get me up to speed.

1. With prior years of weapons inspection & satellite / drone / other air and "boot on the ground" intelligence, how can the pentagon say that many were unknown?

2. What were the Army's / Marine's field orders with regard to what to do when the surrounding area was placed under their control?

3. How many were secured when discovered?

4. How many were left unsecured? For how long?

5. Have there been any charges of dereliction of duty and/or other investigations?

All info is appreciated.
