18. works for me, I can hold hand and have thumb extended.. its a thumb!

perhaps you can more clearly explain this hand position you feel is impossible.. I am holding my hand knuckles pointing forwards and thumb straight up! Then I lift my arm into the position in the video, no problem, feels right for a thumbs up. The important point is the wrist is rotated so the radius and ulna are lined up front to back, so you get the narrower profile of the wrist... like in the video... I can totally see how it looks alot like the finger, but close inspection seems to indicate thumb.
