1. Lyndon LaRouche, but he runs every cycle
I think Bob Kerry, then (D-NE) in the US Senate, was considering a primary run. But he ultimately didn't run.

2. Nope, just a lot of joker candidates
Lyndon LaRouche probably ran as he always does but no serious candidate entered the race.

5. Both Kerrey and Bill Bradley considered it...
But neither did. I think there was some talk about the late Pennsylvania Governor Bob Casey running, as he was still upset about losing a speaking place at the '92 convention supposedly because of his pro-life stand but he didn't run either.
