As you said the downward spiral is really hard to watch...the problem is that even at the worst parts, I'd find myself laughing over the most horrible scenes (the nut with the firecrackers for instance).
One of the things that I particularly enjoyed (and some reviewers had problems with) was that Holmes was already in his downward spiral when the film begins (we don't get to see his "glory days") which is actually unusual for film narratives. Val Kilmer is the most under-rated actor working today. He is much more than just a pretty face.
My best friend thought that it was ruined by a "tacked on happy ending" I disagreed. Any sense of "happiness" at the ending was merely a brief respite before the introduction of crack and AIDS in the porn industry. Things are only going to get worse for the characters. Yes, it is disturbing in how free wheeling everyone was regarding sleeping with anyone....I couldn't believe people actually got it on in the DRIVEWAY! Swingin' 70s I guess. I was a kid for most of the 70s so I don't know.
