2. From what I've read...
It generally starts when the child doesn't bond with the parents, mostly due to neglect/abuse. Said child doesn't learn how to form proper bonds in relationships later in life (unless there is intervention such as therapy to help the child out).

3. I guess I should have that, (my parents were shitty parents) but I
don't. I actually desired the love of other human beings for a long time, and was able to reciprocate it like a normal person. It didn't get significantly paired down until a lot of my friends betrayed me. But it's not like I'm not capable of forming bonds in relationships--I just haven't found anyone new to invest in yet. I guess.
