Do you write like you speak/speak like you write?
I do in terms of tone of voice, level of sarcasm, and vocabulary, as well as sentence structure. In spoken English, however, I add a lot of "like"'s and "er"'s and whatnot, and am a lot more reticent, particularly around strangers or people I'm wary of. My voice is very flat and impassive usually, so I like my "voice" when I write, not when I speak, for in writing I seem a lot more interesting and vibrant. :P

Though in both cases it changes a bit depending on who I am addressing (or talkin' to,) and the matter being discussed.

EDIT to ADD...(after reading more the subsequent posts) I work in the arts, and do speak in front of many ages/demographis of people (nowadays, after being mute as a dancer) and am sometimes sermonizing sort-of, and thankfully because it's arts-related I get to use my points-of-light-speak too. I like the blend!

4. Sometimes I revert back to 'total pothead' mode when trying to explain
something technical. I caught myself doing it the other day, when I was trying to describe how a computer network firewall works with a proxy server and started talking about points of light in an empty black sphere of the unknown

it is typically either a sermon or a lecture.

"likes" and "ers" and other filler words drive me crazy when I hear them used. If I saw them in print I'd probably go insane.

12. My writing is usually more formal--
at least nowadays. Mostly it's political essay that I'm writing now--I haven't yet gotten back "my voice" for writing fiction.

I tend to be more chatty in person, unless I'm with all strangers, in which case I'm quieter. I tend to ramble sometimes. My vocabulary is also more limited when I talk because I don't have time to choose the exact words I'm looking for.

13. Yes, except I say "fuck" less when speaking than I do when writing.
I'm a very good public speaker, if I may say so myself, and I do...that's something not everyone is good at.
