6. Noisemakers. A 12-foot gong would be my first choice.
Of course, the family might not appreciate your gesture, but the kid would love you for life!

10. A new outfit to begin with would be good. Something that doesn't make him
look like a hip-hopper in training.

Giant Legos, as another mentioned.

Dump Truck, Firetruck, or other big piece of machinery.

Candyland game.

Hannibal Lechter playset, with mask and lotion.

Books. Lots and lots of books. "Stinky Cheeseman and other Fairly Stupid Tales" is a great book which he might enjoy, and which you would surely enjoy reading to him.

My First Phosphorous Grenade (box of 12; getting only 1 is just disappointment).

Humidor

Savings Bonds or other investments that will grow nicely until he is 18

Coupon for a free oil change at Jiffy Lube

Train engineer's hat and overalls

Lionel Train Set

Some kind of large photo album/scrapbook so that as he grows up, he can keep adding school photos, letters, notes, souvenirs, etc. to the thing

11. Matchbox Cars
That's all my son has ever wanted since he was nine months old. He's 3 and a half now, and we're swimming in them. Since each one is usually a dollar or less, you can get a whole lot and look like a hero.

or her husband.

I've purchase noisy games, a set of "musical" golf clubs (noise AND home damage), a super soaker ('nuff said) for the kids of a couple where I hate the wife (My SO's obnoxious college roommate & his horrible wife). The kids LOVED their gifts - very satisfying all around.
