10. I would bet that...
...most states would have ridiculously strict laws about transporting beer for delivery to a private residence. Or the pizza place would have to get a special kind of license that the state puts a quota on issuing.

6. So would I and I'm surprised that no one else does it. If it's a profitable
service for pizza places, surely it could be equally so for other types of restaurants.

and you pay a premium for it. It's also usually only available during lunch hour during the week, when businesses do "working" lunches ;)

7. A very old one.
In fact, they were perhaps the first "call and have cheep food delivered" chain of "restaurants" in the country. They had no dine-in restaurants, delivery only. I remember their advertising jingle from the 60's when I lived in New York: "Don't cook tonight; call Chicken Delight."

9. In the late 80's (or was it early 90's?) McDonalds did have delivery
they actually made the delivery guys but golden arches on top of their car

As a matter of fact, even the convenience stores deliver around here. It's amazing.
