I've spent over a decade working in coronary care units and I know pretty much all the devices out there. I think it's much likelier a communications device, probably an amplifier and descrambler that goes along with the earpiece deep in his ear canal. It's meant to be discreet. Ha.
If his problem had been bradycardia (slow heartbeat) instead of alcohol with that pretzel incident, he'd have long since had an internal pacemaker placed. That particular surgery takes only about an hour and a half and could easily have been done during his numerous vacations. The same goes for a defibrillator. External machines are just not reliable, since they'd have to be tweaked every time an event occurred. Plus, that device is simply the wrong shape and size for any of the devices out there. It's not a Holter monitor, either, since those devices are quite a bit bulkier and are generally worn with a shoulder strap so they can be parked from time to time when the patient sits.
Bush's problems are neurological, not cardiac. I think the man has to be stage managed via earpiece during every waking moment.
And that is even sadder than a cardiac problem would be.
