Has anyone heard of Trailblazer Health Enterprises, LLC?
I've gotten 2 Medicare Summary Notices from them for 2 flu shots on Oct. 4, 2008. Amount charged was $55, Medicare did not approve or pay any charges, and I'm not being billed for anything. Oh, and I didn't get any flu shots on 10/4/08.

Is this some kind of Medicare scam or what? Should I just forget about it? I'm confused!

Any suggestions would be greatly appreciated!

1. Could it be somebody scamming Medicare? Billing for fake services?

2. That's what I thought at first but I have no idea.
It refers to "Part B Medical Insurance - Assigned Claims" but on the bottom says "THIS IS NOT A BILL."

I Googled Trailblazers, here: http://www.trailblazerhealth.com/, but I don't even understand the home page!

3. I don't know all of the facts about the mailing you received.
I am licensed to sell life/health insurance in Illinois and I would be glad to help you with any of this. The first thing I would do is contact Trail Blazer and let them know about the error. From what I can tell they have several different phone numbers depending on which state you live in. Did you get a flu shot any time around October of last year? The reason I ask is that some health insurers contract out to smaller (regional) companies to save them money in the long run.

4. They send those notices out so that you can notify them if you did
not receive the services noted. It is their way of combating fraud. If you are sure you did not get the shots let them know.
