Attention Web Site design Gurus. I need help with an on line question form.

I'm trying to design for my site a page similar to this one.

I need my customers to answer questions that I ask them and I need those questions to be delivered to me in some manner via email, etc.

I really don't know how to Google for info on this?

Can you help?

In order to use this, your server would have to support PHP. If your server is running microsoft, however, the answer would be different.

Unless you know or want to learn what's required beyond this, you should ask for more help...
