It advises a person that they are a "target" of a grand jury investigation which may result in the issuance of an indictment.

From an old version of the Dep't of Justice Grand Jury Manual (Nov 1991):

"A 'target' is defined as 'a person as to whom the prosecutor or the grand jury has substantial evidence linking him/her to the commission of a crime am who, in the judgment of the prosecutor, is a putative defendant.' A witness has no right to be told that he is a potential defendant or target of the investigation. The prosecutor has no duty to tell a grand jury witness what evidence it may have against him. Again, however, it is the policy of the Department of Justice to advise a witness of his 'target status if such is the case." (Footnotes & Citations omitted).
