OSHA issues $171,250 in fines
(Florida Keys Keynoter (Marathon) Via Acquire Media NewsEdge) Oct. 18--Four companies have been fined a combined $171,250 for workplace violations that contributed or could have contributed to a construction worker's death at Key West International Airport.
Apex Concrete Construction must pay the most -- $112,700 -- after the U.S. Occupational Safety and Health Administration found that it committed two "willful" and three "serious" violations.
On April 8, Carlos Umberto Fuentes Seanz, 39, was killed when a ramp at the airport's new terminal, under construction, collapsed. Keith White of Big Pine Key was injured.
On one "willful" count, OSHA found that "construction loads were placed on a concrete structure or portion of a concrete structure without ... that the structure or portion of the structure was capable of supporting the loads," therefore "exposing employees to the collapse of concrete and formwork."
On the other "willful" count, OSHA found that "formwork for an elevated ramp had not been designed to support the weight of concrete slab or the weight of precast joists that were place upon it...."
For those two violations, the fine is a combined $98,000.
