It includes all deaths in the armed services regardless of cause or location.

But this report implies to the Gulf War in 1991 and the Iraqi War in the present.

I don't believe they apply to anywhere else.

Read the report carefully.

5. I have read the report carefully
It covers the entire 16 year period from Aug 1990 through 2006. Notice the large number of deaths out of theater. These are all service personnel killed or injured out of theater by all causes.
