1. Reuters reporter has no understanding of dog body language.
So easy to see that he REALLY didn't want a pat on the head. Not that that excuses biting.

2. First of all, you don't come at a dog like that.
I think Barney was spooked by the stuff in the guy's left hand coming towards him. And you shouldn't just jut your hand in a dogs face. You have to hold it palm up, and bring it slowly towards the dog. I really don't blame Barney for taking offense.

3. I usually offer a loose fist
but the approach is the same.

If the dog decides it doesn't want to be friends, it's easier to recover from a bite on the back of a fist than it is to recover from losing a couple of fingers.

4. It certainly didn't help that the handler pulled back on the leash just as the reporter reached.
My dog gets a little freaky when I pull back on the leash in the face of a "threat." She only does it around other dogs but it's a pain in the ass. I guess it would be called leash/dog aggressiveness.
