8. Bring a small sign of some sort.
There's no point in getting in a shouting match with hair-trigger nutjobs like this, but you can make a powerful statement just by staying quiet and holding a sign.

9. Ask them to ask others what their ideas are for fixing healthcare,
and whether everyone should be eligible for MediCare, and then mention that it is govt. run.

Actually, if you can get a number of people to realize that MediCare is govt. run, then hopefully that will be like a pebble in a pond - spreading that fact out far and wide.
