1. More of that black and white thinking
Programs for black lung disease and asbestos diseases belong in a health care bill. States that have previously cut costs in order to expand health care to more people, like Vermont, deserve to have those cuts made up instead of always giving federal money to states that refuse to spend state money on anything.

This is completely different than Ben Nelson's Nebraska gimmick or letting Florida keep Medicare Advantage.

I wish I knew whether these writers were stupid or dishonest, but either way, they aren't helping to resolve one single problem this country faces.

2. Exactly
Most of these examples are things added to make things fair for the Senators' states. It seems from comments in other sources that the solution on the "do gooder" states that expanded Medicaid is that they will change the underlying decision that started the payment help at the number of people currently insured to something that uses the same logic for all states.

The blue states, and the do gooder states are mostly blue, already pay far more in as taxes and get less back. In the past, they have voted for what helps the country, even though it hurt their state. Here, Leahy, Sanders and Kerry, stood up to give VT and MA their fair share - not more than their fair share.

All else, except for payoffs from the lobbyists, come in second.

A very distant second.
