Ventilator Shafts.
Why doesn't this mine already have ventilator shafts? I think it would be a standard procedure to punch ventilator shafts into the mine at several locations across its length.

1. They do, and they can collapse when the mine explodes
When there hasn't been an explosion, they ventilate the mine. When an explosion has sealed off parts of the mine away from the shafts, the methane keeps building up.

but the explosion collapsed a few of them I am betting

Now let me add and this is pure speculation... if they were not clear, that leads to buildup. and if they were not enough, same story.

I am speculating here, but I am almost willing to bet that when the investigation is done. assuming it is not white washed, a combo of these problems will occur, given some of the reported violations.
