If you work in industry, mining or construction, you're familiar with the daily safety talk and the constant hunt for near-misses. Some companies require supervisors to identify and address so many near misses a month. Here's the idea behind it:

Depending on who is doing the counting, for every so many near misses(something that could have been an accident and wasn't), you get a recordable incident (someone ends up going to First Aid)

For every so many recordables, you get a lost time accident (someone misses work)

For every so many lost time accidents, you kill someone.

It's a rule of thumb that holds true; by cutting back on near misses you prevent deaths. By tracking near misses, you can get a handle on the level of safety in your operation.

SO

How many blood and thunder speeches before people start talking about guns and revolution?

How many people talking guns and revolution before someone throws a brick through an office window?

How many bricks through office windows before you get death threats?

How many death threats before someone sets off a bomb?
