I'm pretty sure that Pelosi voted for herself. I understand that Costa and Cardoza voted for each other... which it almost certainly a deal to vote for yourself without voting for yourself.

I thought elections were necessary because more than one person is a candidate.

the dems are supposed to vote for the dem and the pubs for the pub. The election takes place within the caucuses.

heath and anyone else who wanted to were perfectly able to run against her in the caucus. He did, as a matter of fact. He lost, fair and square. After that it is time to show unity. To put this in perspective. If the majority were 218 to 217, if he had pulled this stunt we would have a GOP speaker even though we have a majority of Democrats.

20. The election ultimately takes place on the House floor
But, in the past few decades at least, the expectation is that the vote will be along party lines with the designated candidate of each caucus splitting the vote. If you check the votes for speaker over the past 20 years, you'll see that the vote generally is split between two candidates, one Democrat, one Republican. There usually are a couple of no shows and/or a couple of folks who voted present. In 1997, around eight or nine repubs either voted for someone other than Gingrich or voted present. And between 2001 and 2005, Gene Taylor voted for Murtha rather than for the designated caucus nominee (Gephardt or Pelosi).
