5. No, not in the UK of GB and NI.
You just need an address and proof that you live at that address, i.e. A bank statement that has your name and adrdress on it. I first went to England as a US student and had no problems once so ever getting on the National Health Service. A few months later I got a British passport and thus had proof of my UK. Citizenship through decent (my mum is British, therefore so am I). There was no difference in treatment between being an Anerican and being a British citizen. In the British Home Countries, health care is a right and as such nationality and wealth status are non-issues. If anyone here is in dire need of treatment, may I suggest renting a place in the UK and living there for a given amount of time for treatment. Yeah it'll cost £10,000 to stay there, but that may be cheaper than paying for health 'insurance'. If you do take this route, don't try and get a job...that's very tricky and if you stay longer than six months you may encounter problems.
