https://www.nytimes.com/2018/10/08/nyregion/prestige-limousine-crash-schoharie.html
A driver with an improper license. A limousine company with a trail of failed inspections and ties to a scheme to illegally obtain drivers licenses. And a limousine itself that had also been deemed unsafe.
Two days after a devastating limousine crash in upstate New York that killed 20 people, officials revealed new details about their inquiry that suggested that the trip never should have been allowed to happen.
The mounting questions about the accident increasingly centered on the limousine company, Prestige Limousine, which had a shoddy record, operated out of a back room in a low-budget hotel and had a history of suspicious dealings that seemed to extend to Dubai. On Monday, officials moved to suspend the companys operations and seize its vehicles.
<snip>
The owner of the limousine company, Shahed Hussain, has the same name and address as that of a former informant for the F.B.I. who has testified in two prominent terrorism cases, according to public records. A law enforcement official suggested that his son may operate the limousine company.
The company is based at the Crest Hotel and Suites in Gansevoort, N.Y., a small town north of Albany, and on Monday, state troopers were at the hotel. The State Police said they had seized three vehicles, including another limousine, from the company and believed that Mr. Hussain was outside of the United States.
Arnie Cornett, the manager at the hotel, confirmed that the State Police were looking for the owner of the hotel and limousine company. Mr. Cornett identified the owner only as Malik and said he lived in Dubai. Mr. Hussain, the informant, went by Malik when he helped the F.B.I. infiltrate a mosque in Albany.

Confidential Courier Service, Secure Car Service, div. Mac's Taxi.
Seriously, the whole "industry" operates in a loophole zone, and there are plenty of shaky players. Or it could be that it had a simple consumer-grade brake failure, while vastly overloaded!
2 tons of people, plus maybe 1.5 tons of stretch and limo bling, added to a big SUV chassis which is already near capacity on its brakes and tires, and a small failure can cascade, aka "go bad on you quick!"
The Limo owner's status r/e Federal Confidential Informant is interesting. Those sorts of folks are often in need of killin`....

Queensbury man, Fort Ann native mourned in fatal limo crash
Both the driver and one of the passengers in the countrys deadliest transportation accident since 2009 lived in the area, and both are being mourned now.
The limousine driver in Saturdays deadly crash lived in Queensbury, and friends are raising money for his funeral.
Scott Lisinicchia was driving the limo that crashed in Schoharie, killing 20 people.
He was married to Kim Green-Lisinicchia, who for years provided free makeovers in Glens Falls through her nonprofit, Fashion for Prosperity. It was designed for victims of domestic violence, people who were homeless and those who had achieved sobriety after addiction.
Green-Lisinicchia posted the GoFundMe link on Facebook before saying she would be leaving Facebook while she grieved.
I want to thank you all for your prayers and comfort, she wrote. It hurts me to a core to have to bury my husband. I miss him so very much ... I love you Scott.
The GoFundMe campaign for him can be found at https://www.gofundme.com/68a0zs8/. It was organized by a friend who only identified herself as Thea.
All money raised will go toward funeral expenses. As of Monday afternoon, $650 had been raised.
