I've spent 28 years now married into a Cuban-American family in Miami and that's not how anyone I know would take it. To make papaya refer to vagina around here you'd have to put context around it that made it work that way. Just in a list of fruits, talking obviously about fruits? No way, sorry.

This is about like using the word brownie in the US. In a list of things served at a dinner, not really any joke there. But make it brownies, and add a wink, maybe some air quotes, now you've made a joke.
