....despite its seemingly suggestive nature, "Balls to the Wall" is actually a benign and innocent phrase.

It originated from aviation days (others claim it was earlier and related to trains), where the throttles had ball shaped handles, and pressing them forward towards the "wall" of the panel meant you were giving the craft maximum energy to go forward.

It refers to the ball tops of airplane engine controls.
Push them forward all the way -- to the wall -- to max out all engines.
