Our flu shots at the facility I work at are pre-packaged, and we have to fill out lot number, manufacturers, expiration date etc. we also use a screening questionnaire. The origins of this should be easy to track.

Insulin overdose is never going to look like flu. For one thing, where is the fever? We are talking about sweats, tunnel vision, shakes, changes in vital signs, headaches and loss of consciousness.
