He believes his own narrative about about himself. It is part of his mental illness.

He doesn't believe he can fool all the people all the time. He believes all the people agree with him all the time because he is right (even if what is "right" today is the opposite of what was "right" yesterday). Anyone who doesn't support the narrative, however irrational the narrative may be, is "nuts" or "wrong" or evil" or "nasty." There is something wrong with them. Not with him.

He is always flabbergasted that to find "some of the people" aren't buying.

His narcissistic belief in the awesome power of his awesomeness knows no bounds.

And the massive defenses he has in place to protect his eggshell ego keep any thoughts of fallibility out.
