I believe the biggest threat to his life is what any REAL doctor will have told him if he ever got himself examined by one. He is seriously overweight, and his diet has to mean that he has some serious arterial blockage, even if his cholesterol count is not in the danger zone yet.

Men in better health than he have dropped dead of sudden heart attacks, and at a younger age. The fact that he hasn't had one yet does not guarantee he won't have one in the next five hours.
