Offer Medicare for all as an option (ramp up the infrastructure) create a federal exchange open to everyone (for the transition) and move up the date for state waivers (benefit to states like Vermont).

This would be the fastest route.

at the state level might be the stepping stone. At least it's an opportunity for the feds to shine and move closer to single payer, maybe even in those very states.

http://www.nytimes.com/2012/10/28/health/us-to-sponsor-health-insurance-plans-nationwide.html

The Obama administration will soon take on a new role as the sponsor of at least two nationwide health insurance plans to be operated under contract with the federal government and offered to consumers in every state.

These multistate plans were included in President Obama’s health care law as a substitute for a pure government-run health insurance program — the public option sought by many liberal Democrats and reviled by Republicans. Supporters of the national plans say they will increase competition in state health insurance markets, many of which are dominated by a handful of companies.

Makes sense to me, but Washington, D.C. likes to be the reverse Robin Hood and funnel money to the rich. It loves reverse socialism.

I'm hoping that the several/many Republican Governors who are refusing to implement the exchanges, thus leaving the Feds to do so for them, don't realize they are helping to get there sooner.

Certainly, they are not that stupid?

Or maybe they really recognize the benefits of universal, single payer, and this is how they get the country there while still maintaining their public opposition.
