You can register to vote while enrolling (like at the DMV when you're getting a driver's license).
The poor are the ones that benefit the most from the ACA, and they're also the demographics that vote the least. The common wisdom is that this group would be heavily D in their voting if they ever turned out, and boom here's a thing likely to drive their turnout through the roof.
The hate is fake, it's really about keeping the poor from voting Democratic.

and hospital facilities with those scruffy, poor people. I have actually had this said to me by someone who believes that's why we have county hospitals. "If they get health insurance they'll be in our hospitals and seeing our doctors."
