It's mean if I say your mother wears combat boots. But it's protected by the first amendment. You can throw me out of your house for saying it, but you can't have me arrested for it. For another example, the Republicans can stomp their feet and whine that the media is mean when they ask questions. But they can't call the police on the media for it.

It's a threat if I say I wish to harm a specific individual or group.

These are very different things. Nobody said that an actual threat was protected speech.
