There is a disconnect at some basic level where he just seems incapable of feeling any emotion or compassion for the people.

How about areas nearly impossible to reach? Aren't there any diabetics there? There certainly isn't any usable insulin any more (no way to refrigerate it). How about cholera? Infections from injuries? Nobody died of thirst because there's no potable water?

I sincerely hope that none of the above happens, but weeks of neglecting maladies will take its toll.
