They added the scene where Sally fakes an orgasm in a crowded restaurant to give Meg Ryan a chance to show her comic abilities. Estelle wasn’t supposed to have a speaking role, but, just before filming, Billy Crystal suggested the line.

I have a passion for ad libs and other unscripted scenes from movies. Some of the most famous lines in cinema history weren’t in the script. I keep a (very long) list. Maybe one of these days I’ll post it here.
