1. I'm guessing it ran the red light but we couldn't see it, it was blocked by the car in front of us.

It still looks a bit mysterious but apparently the 'phantom' car entered the intersection between the white box-truck and the trolley and did so while our view of it was blocked by the guy he almost got hit by.
