have a clue the survey is available- certainly not because of a great effort by US bishops.

I did cross post in the Catholic/Orthodox group.

I didn't click on it, but does it ask if you are catholic or not?

For some reason, I thought they were doing this at the parish level.

I can't tell you exactly. It asks City, State and what capacity, priest, deacon, lay, chaplain, parent etc. It doesn't ask parish or whether you are Catholic.

As to the parish level- not my parish and none of the others in my area- I will update if I am wrong.

The article that was posted here about it said it specifically wanted to get the opinions of catholics, but if it's generally available it makes me wonder if they will dismiss it as not truly reflective of catholics.

In the UK the Bishops posted it on their web site and they ask what parish you belong to. Come to think of it this one ask what diocese.
